The best way I've found to do a really fabulous extraction is to use the freehand select tool. I use PaintShopPro so I don't know what the tool is called in Photoshop.
It's a simple procedure though it does take some patience. I draw around the parts that I want to get rid of and then invert the selection, copy the image that I want and then paste it as a new image or into another workspace that I've got open.
If you are using a mouse with your computer you'll be zooming in very close when you draw your selection. Even with a Wacom pen I still zoom in for the cleanest edges on my extraction.
